ABSTRACT

Objetivo: analisar a importância da enfermagem no manejo da bolsa de colostomia em pacientes com câncer colorretal, buscando identificar o papel da enfermagem no processo de viver da pessoa ostomizada com câncer. Método: trata-se de uma revisão da literatura realizada nas bases de dados SCIELO, LILACS e BDEnf, realizada entre abril e maio de 2022 com os descritores bolsa de colostomia, câncer colorretal, enfermagem. Os artigos selecionados para análise e interpretação tinham como os seguintes critérios de inclusão: textos completos sem restrição de delineamento ou cronograma do estudo; escrito em português, disponibilizado em bases de dados gratuitamente e que contemplam o objetivo da pesquisa. Já os critérios de exclusão foram: artigos de revisão bibliográfica, com publicações anteriores a 2016 e aqueles que não contribuía para enriquecer a discussão do tema proposto. Assim, foram selecionados 10 artigos, que serviram de base para a discussão. Resultado: após a análise dos artigos foram encontrados os seguintes temas para discussão: câncer colorretal e a estomização, assistência de enfermagem à pacientes com colostomia e as percepções de pacientes estomizados com câncer colorretal. Conclusão: a sistematização da assistência de enfermagem ao estomizado e sua família é essencial para sua reabilitação, para sua autonomia e o pleno exercício de dignidade diante do enfretamento de uma doença tão estigmatizada que é o câncer colorretal.


Objective: to analyze the importance of nursing in the management of the colostomy bag in patients with colorectal cancer, seeking to identify the role of nursing in the life process of the ostomized person with cancer. Method: this is a literature review carried out in the SCIELO, LILACS and BDEnf databases, carried out between April and May 2022 with the descriptors colostomy bag, colorectal cancer, nursing. The articles selected for analysis and interpretation had the following inclusion criteria: full texts without restriction of study design or schedule; written in Portuguese, made available in databases free of charge and covering the purpose of the research. The exclusion criteria were: literature review articles, with publications prior to 2016 and those that did not contribute to enrich the discussion of the proposed topic. Thus, 10 articles were selected, which served as the basis for the discussion. Results: after analyzing the articles, the following topics were found for discussion: colorectal cancer and ostomy, nursing care for colostomy patients and the perceptions of ostomized patients with colorectal cancer. Conclusion: the systematization of nursing care for people with stomas and their families is essential for their rehabilitation, for their autonomy and for the full exercise of dignity in the face of facing such a stigmatized disease that is colorectal cancer


Objetivo: analizar la importancia de la enfermería en el manejo de la bolsa de colostomía en pacientes con cáncer colorrectal, buscando identificar el papel de la enfermería en el proceso de vida de la persona ostomizada con cáncer. Método: se trata de una revisión bibliográfica realizada en las bases de datos SCIELO, LILACS y BDEnf, realizada entre abril y mayo de 2022 con los descriptores bolsa de colostomía, cáncer colorrectal, enfermería. Los artículos seleccionados para análisis e interpretación tuvieron los siguientes criterios de inclusión: textos completos sin restricción de diseño de estudio o cronograma; escrito en portugués, disponible en bases de datos de forma gratuita y que cubre el objetivo de la investigación. Los criterios de exclusión fueron: artículos de revisión bibliográfica, con publicaciones anteriores a 2016 y aquellos que no contribuyeron a enriquecer la discusión del tema propuesto. Así, fueron seleccionados 10 artículos, que sirvieron de base para la discusión. Resultados: después del análisis de los artículos, se encontraron los siguientes temas de discusión: el cáncer colorrectal y la ostomía, la atención de enfermería a los pacientes con colostomía y las percepciones de los pacientes ostomizados con cáncer colorrectal. Conclusión: la sistematización de los cuidados de enfermería a las personas con estomas y sus familias es fundamental para su rehabilitación, para su autonomía y para el pleno ejercicio de la dignidad frente al enfrentamiento de una enfermedad tan estigmatizada como es el cáncer colorrectal

ABSTRACT

Purpose: To elucidate the actual state of difficulties and related factors of nurses in caring for patients with recurrent and metastatic malignant musculoskeletal tumor. Methods: The subjects were 315 nurses from 12 institutions that treat malignant musculoskeletal tumor. We researched the difficulties of nurses regarding cancer nursing and care of patients with malignant musculoskeletal tumor. We examined the related factors by multiple regression analysis. Result: 165 nurses responded (effective response rate 52.4%). Among “Nurses’ difficulty with cancer care”, the level of difficulty regarding communication was the highest and in own knowledge/technology, system/regional cooperation was also high. Among “Nurses’ difficulty with care for patients with malignant musculoskeletal tumor”, there was a high degree of difficulties regarding the target characteristics of care. Related factors were facility characteristics, nursing experience, nursing experience of musculoskeletal tumor, learning methods for musculoskeletal tumor nursing, and multi-professional conferences. Conclusion: Nurses caring for patients with malignant musculoskeletal tumor have specific difficulty due to total pain, age of onset, and rarity. Educational support based on related factors is therefore required.

ABSTRACT

Purpose: To clarify current status of nurses in support of children with parents with cancer in Akita Prefecture. Method: Questionnaire surveys were administered on nurses in Akita prefecture. Their experiences, educational backgrounds, and ability to recognize the need for support were examined. Descriptive statistics were used for the attributes, and chi-square test or Fisher’s exact Test or Wilcoxon rank sum test was used to examine the relationship between the support experience and the attributes (p<0.05). The contents of their open comments were then analyzed using a qualitative integration method. Results: Responses were obtained from 141 nurses at 10 facilities (collection rate: 43.9%). Of the 141 nurses, 135 (96%) recognized the necessity of support for children, and 28 (20%) already had experience supporting them. The contents of support were [direct support for children] [support for children through parents] [support through coordination and coordination with resources]. Most apparent difficulties in support were “Intervene for children” and “to have a connection with children”. Conclusion: The results indicate that nurses recognize that children need support. However, they lack experience in providing such support. It is necessary to enhance support by learning support methods and sharing information with other occupations. Support from parents is important when children cannot be met.

ABSTRACT

Objective: To clarify administrators' awareness of outpatient cancer nursing clinics.Methods: We conducted semi-structured interviews of six administrators overseeing outpatient cancer nursing clinics and performed a qualitative, inductive analysis.Results and discussion: In this study, interviews with the administrators led to the identification of ten categories for outpatient cancer nursing clinics such as [the nurses' role in easing the concerns and worries of patients with cancer and their families], [challenges in training and securing outpatient cancer nursing clinic personnel], and [the influence of the passion and confidence of nurses on practice in outpatient cancer nursing clinics]. These interviews revealed that, in practice, outpatient cancer nursing is a useful skill for elucidating the real needs of patients and supporting their decision-making. Coordinated efforts and collaboration among human resources/disciplines may affect day-to-day outpatient cancer nursing clinics.Conclusion: This study revealed that the development of assessment indices to substantiate the accomplishment of outpatient cancer nursing clinics and the training of personnel with a high level of practical skills are two challenges for the expansion of outpatient cancer nursing clinics.

ABSTRACT

The purposes of this study were to compare the response distribution of the result of using “nurse’s difficulty scale with cancer care” to nurses engaged in cancer nursing with 2010 survey at Tohoku University Hospital. Self-reported questionnaires were distributed to 512 nurses in 2010 and to 524 nurses in 2016. Responses from 356 (70%) nurses in 2010 and from 332 (63%) nurses in 2016 were analyzed. As a result of comparing the surveys of 2010 and 2016, the rate of correct answer in each items of the knowledge scale rose significantly (all p<0.05). From the items regarding their own knowledge and skills, difficulty in collaboration with the community, and end-of-life care decreased (both p<0.05). Despite the increase in the correct answer rate of the knowledge scale, the difficulty has become high. This may reflect the complexity in cancer care due to development of treatment.

Resumen:


Objetivos: analizar a través de una “situación de enfermería” el cuidado integral enfermero en cuidado intensivo oncológico pediátrico. Método: con base en la guía para el análisis de situaciones de enfermería desarrollada por el Grupo de Cuidado al Paciente Crónico y su Familia, de la Universidad Nacional de Colombia, se tomó una historia que sucedió en una unidad de cuidado intensivo oncológico pediátrico, para identificar en ella los componentes teórico-conceptuales del cuidado integral de enfermería.


Resultados: la “situación de enfermería” analizada refleja la visión filosófica integrativa-interactiva de la enfermería, permite analizar los conceptos que se evidencian en la práctica clínica y explorar los conocimientos que exige el cuidado integral de un paciente pediátrico con cáncer en cuidados intensivos, donde se ponen en evidencia los principios y valores profesionales e institucionales. Conclusión: la “situación de enfermería” vivida con una niña atendida en cuidado intensivo oncológico pediátrico y su familia, permite comunicar los conocimientos de cuidado que requiere una enfermera. La aplicación de una guía para reflexionar sobre la propia práctica clínica facilita el análisis y comprensión del cuidado, así como los valores y principios que lo respaldan.


Abstract:


Purpose: to analyze integral nursing care through a "nursing situation" in pediatric cancer intensive care. Methods: based on a guideline for nursing situation analysis developed by the Group for Chronic Patients and Families Care at National University of Colombia, a narrative was used in a pediatric cancer intensive care unit, in order to recognize theoretical-conceptual elements in integral nursing care. Results: this “nursing situation” reflects an integrative-interactive philosophical view of nursing and allows concepts shown in clinical practice to be analyzed. Moreover, knowledge required by integral care of pediatric cancer patients in intensive care units is explored with principles and professional and institutional values being shown.


Conclusion: a “nursing situation” in a girl being admitted in a pediatric cancer intensive care unit and her relatives allows knowledge on nursing care to be communicated. Use of a guideline to think about nursing clinical practice helps to analyze and understand both nursing care and underlying values and principles.


Objetivo: para analisar através de uma "situação de enfermagem" cuidados de enfermagem globais em oncologia pediátrica de tratamento intensivo. Método: com base na orientação para a análise de situações de enfermagem desenvolvido pelo Grupo de Assistência ao Paciente Crônico e Família da Universidade Nacional da Colômbia, uma narrativa que aconteceu em uma unidade de oncologia pediátrica de terapia intensiva foi feita para identificar que componentes teóricos conceituais de cuidados de enfermagem globais. Resultados: o "status de Enfermagem" reflete analisados integrativa visão filosófica de enfermagem interativo, para analisar os conceitos que são evidentes na prática clínica e explorar o conhecimento necessário cuidado integral aos pacientes pediátricos de câncer em cuidados intensivos, onde Eles destacam os princípios e valores profissionais e institucionais. Conclusão: "situação de Enfermagem" vivia com uma menina tratada em oncologia de terapia intensiva pediátrica e sua família, podem se comunicar conhecimento dos cuidados exigidos por uma enfermeira. Aplicando um guia para refletir sobre a prática clínica própria facilita a análise e compreensão dos cuidados, bem como os valores e princípios que a sustentam.

ABSTRACT

Objective To research the significance of application of clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstruction.Methods Sixty patients with tongue cancer who undertook glossectomy and forearm flap reconstruction were enrolled.The patients were randomly divided into the experimental group and the control group (30 cases in each group).The patients of the experimental group received nursing following clinical nursing pathway,and the patients in the control group received routine nursing service.The patients and doctors satisfaction evaluation were compared between the two groups with a self-designed nursing quality evaluation form.The qualities of care achieved in 2 groups were compared.Results It took less in-ward time and expenditure in the experimental group compared with the control group.The quality of care,the satisfactory degree of patients and doctors in the exipermental group were significantly higher than those of the control group.Conclusions The establishment and application of the clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstrucition contributes to improve the satisfaction of patients and doctors and the quality of nursing service.

ABSTRACT

The purposes of this study were to describe nurses’ difficulty with cancer care and explore factors related to the difficulty of cancer care. Self-reported questionnaires were distributed to 512 nurses who are engaged in cancer care at Tohoku University Hospital. Responses from 344 (67%) nurses were subjected to analysis. The results revealed that nurses felt that “communication” was the most difficult aspect of cancer nursing followed by “hospital system and regional alliances” and “knowledge and skill.” Nurses did not feel that “collaboration with doctors” “disclosure and explanation of disease” or “death and dying” had particularly high levels of difficulty, however, their levels of difficulty could be improved. Greater difficulty with cancer care was reported by nurses working on the general ward and nurses with limited cancer care experience in the last year. Communication skill training,education (especially for less experienced nurses), expansion of palliative care, and restructuring of discharge planning and regional collaboration systems might contribute to decreasing nurses’ difficulty with cancer care.

Objetivo: analizar a través de una “situación de enfermería” y bajo los planteamientos de la “enfermería como cuidado”, los enfoques teórico-conceptuales que respaldan a la disciplina profesional de enfermería. Método: con base en la guía para el análisis de situaciones de enfermería desarrollada por el Grupo de Cuidado al Paciente Crónico y su Familia de la Universidad Nacional de Colombia, se tomó una narrativa de una experiencia de la práctica clínica del servicio de admisiones del Instituto Nacional de Cancerología ESE de Colombia, para identificar los componentes teórico conceptuales de la disciplina profesional a la luz de los planteamientos de la enfermería como cuidado. Resultados: la situación de enfermería que refiere la narrativa presentada muestra los conceptos metaparadigmáticos y los diferentes conocimientos que requiere una enfermera para el cuidado en un servicio de admisiones. Conclusión: en la situación de enfermería se describe la experiencia de cuidado vivida por una enfermera en el servicio de admisiones y ratifica la importancia de contar con un profesional de enfermería en este servicio.


Abstract:


Purpose: To analyze by means of a "nursing scenario" and with an approach of "nursing care", theoretical conceptual approaches supporting professional nursing tasks.


Method: Based on guidelines for nursing scenarios analyses previously developed by the Grupo de Cuidado al Paciente Crónico y su Familia at the Universidad Nacional de Colombia, an experience from clinical practice at the admission service, Instituto Nacional de Cancerología ESE, Colombia, aimed at identifying theoretical conceptual items of a professional nursing task was described, taking into account nursing care-oriented approaches.


Results: A nursing scenario refers to a description of meta-paradigmatic concepts and the knowledge a nurse must have to rule an admission service.


Conclusion: In the nursing scenario, care-oriented experience of a nurse in an admission service is reported, and the importance of a professional nurse presence in such a service is highlighted.


Objetivo: analisar através de “uma situação de enfermagem” e abordando a “Enfermagem como cuidado” os enfoques teórico conceituais que apóiam a disciplina profissional da enfermagem. Método: com base no guia para a analise de situações de enfermagem desenvolvido pelo Grupo de Cuidado ao Paciente Crônico e a Sua Família da Universidade Nacional de Colômbia, tomou-se uma narrativa de uma experiência da pratica clinica do Serviço de Admissões do Instituto Nacional de Cancerologia ESE de Colômbia, para identificar as componentes teóricas-conceituais da disciplina profissional, a luz da abordagem de enfermagem como cuidado. Resultados: a Situação de enfermagem referida pela narrativa apresentada mostra os conceitos metaparadigmaticos assim que os diferentes conhecimentos que requer uma enfermeira para administrar cuidados num serviço de admissões. Conclusão: nesta situação de enfermagem, é descrita a experiência de cuidados vivida por uma enfermeira no serviço de admissões e é acrescentada a importância do papel de uma profissional de enfermagem neste serviço.

ABSTRACT

O estudo teve como objetivo analisar a produção científica da enfermagem na atenção paliativa oncológica em unidades de terapia intensiva. Pesquisa exploratória, descritiva, retrospectiva e bibliográfica de 23 artigos produzidos de 2000 a 2010. O referencial teórico-metodológico foi a Metodologia de Categorização Epistemológica para a Pesquisa na Enfermagem com ênfase na constituição do tema ou problema da pesquisa. Os resultados indicam que o conhecimento produzido concentra-se no âmbito internacional, setorizado predominantemente na unidade de terapia intensiva adulto. A análise dos temas focalizados indica a apreensão do fenômeno na esfera subjetiva, na ótica dos enfermeiros como sujeitos/consciência do conhecimento para apreensão de situações de enfermagem relacionadas às estratégias e obstáculos à implantação da atenção paliativa oncológica nesse cenário, além das contribuições da atenção paliativa oncológica para clientes e familiares. As repercussões demonstradas nos aspectos epistemológicos destacados possibilitam afirmar que a assistência de enfermagem a pessoas com câncer avançado sem possibilidades de cura na Unidade de Terapia Intensiva é permeada por desafios que requerem investigações para subsidiar critérios e estratégias para atuação da equipe de enfermagem para essa clientela.


The study had as goal to analyze the scientific production of nursing in the cancer palliative care in the intensive care units. It is an exploratory, descriptive, retrospective and bibliographic search of 23 articles produced from 2000 to 2010. The methodological - theoretical reference was the Methodology of Epistemological Categorization for the search in the Nursing with emphasis in the constitution of the theme or of the resource problem. The results show that the produced knowledge is concentrated in the international field, predominantly in the adult intensive care sector. The analysis of the focused themes shows the phenomenon comprehension in the subjective sphere, in the nurses' view as people/awareness of the knowledge for the understanding of Nursing situations related to the strategies and obstacles to the implantation of cancer palliative care in this field beyond the contributions of the cancer palliative care to customers and family. The repercussions demonstrated in the eminent epistemological aspects enable to assert that the Nursing Care to people with advanced cancer without possibilities of cure in the intensive care unit is full of challenges which demand investigations to subsidize criterions and strategies for performance of the Nursing team with these customers.


El estudio tuvo como objetivo analizar la producción científica de enfermería en la atención paliativa oncológica en Unidades de Terapia Intensiva (UTI). Investigación exploratoria, descriptiva, retrospectiva y bibliográfica de 23 artículos producidos entre 2000 y 2010. El referencial teórico-metodológico fue la Metodología de Categorización Epistemológica para Pesquisa en Enfermería con énfasis en la constitución del tema o problema de la investigación. Los resultados indican que el conocimiento producido se concentra en el ámbito internacional, sectorizada predominantemente en la unidad de terapia intensiva adulta. El análisis de los temas focalizados indica la aprehensión del fenómeno en la esfera subjetiva, en la óptica de los enfermeros como sujetos/consciencia del conocimiento para aprehensión de situaciones de enfermería relacionadas a estrategias y obstáculos a la implantación de la atención paliativa oncológica en ese escenario, más allá de las contribuciones de la atención paliativa oncológica para clientes y familiares. Las repercusiones demostradas en los aspectos epistemológicos destacados posibilitan afirmar que la asistencia de enfermería a personas con cáncer avanzado sin posibilidades de cura en la unidad de terapia intensiva es permeada por desafíos que requieren investigaciones para subsidiar criterios y estrategias para la actuación del equipo de enfermería junto a esa clientela.

ABSTRACT

PURPOSE: The purpose of this study was to explore the experiences of gastric cancer couples in Korea and to generate a substantive theory integrating the experiences of gastric cancer survivors and their spouses as a whole. The specific aims of this study were to explore major problems gastric cancer couples faced and how they resolved these problems, focusing on inter-relational dynamics within the couples and on similarities and differences between the couples. METHODS: This was a secondary analysis study using grounded theory techniques. The study used the data of 11 married couples which was collected from in-depth interviews from two primary studies. The unit of analysis was dyads of gastric cancer survivors and their spouses. RESULT: The basic social psychological process that emerged from the analysis was "taking charge of their health." Major categories involved in this process were identified as 1) adjusting to new diets, 2) reinforcing physical strength, 3) seeking information, 4) strengthening Ki, 5) lowering life-expectations, and 6) going their separate ways. These six categories represent major strategies in overcoming critical problems that occurred in day-to-day experiences. In terms of the process, the first five categories characterize the earlier stage of the process of "taking charge of their health," while "going their separate ways" indicates the later stage and also the beginning of their separate ways: "pursuing spiritual life" for the survivors, and "preparing for the future" for the spouses. CONCLUSIONS: The results of this study will help design family care for the people with gastric cancer by providing in-depth understanding and insight on the lives of gastric cancer couples.
